INSTRUMENTS
It is recommended that Piano and Guitar students have their own instruments at home to practice, but this is not required to participate in the program.

TUITION FEE
Registration is $20.00 per student, if you are accepted into the program. In addition, there is a fee of $5.00 for each class, which must be paid weekly.

SINGING EVALUATION / TRY OUT (VOICES)
Students who want to take singing classes will need to go through an evaluation before being accepted.

BY SIGNING THIS FORM:
I commit to and understand the guidelines and policies stated therein.
I am the parent/legal guardian of the child and have the legal authority to act on behalf of the child.
All the information provided in this form is true and correct to the best of my knowledge.
I have been advised to update any changes that may occur after my signing and delivery of this form, and I take responsibility for any situation that may arise if I do not update any and all necessary changes.
I give Time to Change Generations (TTCG) permission to take photos and/or videos and understand that these may be used for advertising purposes.
I understand that the services provided by Time to Change Generations (TTCG) are offered to the community, and my use of these services is solely my decision.
I release Time to Change Generations (TTCG), its volunteers, and/or coordinator(s) of any responsibility and liability of any claim whatsoever that arises or may arise during and after my use of their services.
This form does not guarantee acceptance into the program and Time to Change Generations (TTCG) reserves the right to deny enrollment to any student, even after being accepted, if they do not adhere to the established rules.
